Conditions for obtaining service
- The beneficiary should not be benefiting from any other health insurance program.
- The age of the beneficiaries (if sons) shall be under (18) years old.

Royal Medical Services to verify the coverage of the beneficiaries under the military health insurance (parents, brothers, sisters and their husbands)
Procedures
- The applicant shall obtain the approved application form from the Reception Desk and fill it out.
- The applicant shall then submit the form and the enclosed documents to the Reception Desk officer.
- If the wife wishes to add the husband or the subscriber wishes to add one of his/her parents, then the Royal Medical Services Liaison Officer must stamp the application form.
- The form and the enclosed documents will be verified for completeness.
- If the submitted documents are not complete, the applicant will be asked to update the required documents.
- If the submitted documents are complete, the applicant will receive a payment voucher showing the due amount.
- The auditor endorses the voucher.
- The applicant pays the prescribed fees at the cashier’s counter.
- Submit the form and the enclosed documents to the Reception Desk officer
- The Applications Intake officer enters data.
- The auditor receives the application and validates information against it.
- In case of errors, the application is sent back to the Applications Intake officer to double check the entered dat
- In case the application is complete, the Audit and Posting officer will post the application.
- The application is sent electronically to printing department.
- The new card is printed and handed over to the Reception Desk officer.
- The Reception Desk officer then gives the new health card to the applicant.
Required Documents
- A cover letter from the ministry, institution or the authority in which the applicant is an employee.
- The Employee Appointment Decision or the final employee’s salary structures.
- A recent salary statement showing the deduction for the health insurance allowance.
- A copy of the Family Book.
- One (1) personal photograph for every beneficiary and subscriber.
Persons benefiting from the service
The employees of government departments, municipalities, public institutions and official bodies, their wives or husbands and children under (18) years.
